Questionnaire

• The Questionnaire function allows the Chairmanship holder to issue a questionnaire to every participants and count the result of the votes from the partcipants.
• Select “Questionnaire” in pop-up menu of the Local View of the Chairmanship holder to create a Questionnaire. The participants other than the Chairmanship holder cannot use this function.
• When the Chairmanship holder closes the responses, the Questionnaire function ends.

<Create a questionnaire>



	Question	Enter the question within 40 characters.
	History	The history of choices is saved in 12 times. Choices can be selected from the history.
	Choices	Enter the choices within 20 characters.
	Allow multiple selections	Allow the participants to vote to the multiple choices.
	Anonymous voting	User ID and names are not displayed on the Result of the Questionnaire.
	Close button	Close this window.
	Issue button	Issue a questionnaire to every other participants.

• A questionnaire without question sentence is allowed to issue.
• A questionnaire with the same choices or only one choice is not allowed to issue.

<Answer a questionnaire>



	Answer	Check the Choices to answer.
	Submit	Submit the answer.

• Once the questionnaire is answered, it cannot be changed or canceled.
• If a participant enters the room while the questionnaire is running, this window will be shown.
  (This window will not be shown if the participant has already submitted the same questionnaire.)
• After the Chairmanship holder clicks “Close response” button or closes the “Result of the questionnaire” window, the questionnaire is closed and cannot be answered.

<Result of the questionnaire>



	Result	Displays the result of the questionnaire.
	Respondent	Place the mouse cursor over the choice to display the user ID and the name who selected it.
	Close response button	Close response for this questionnaire.
	Response rate	Display the response rate of the issued questionnaire. (Respondents/Participants who had received the questionnaire.)
	Save button	Save the result in CSV format.
	Close button	Close this window.

• The result of the questionnaire is updated in real-time.
• The Questionnaire is aborted if the Chairmanship holder leaves the room before closing response.
• The ratio to each choices becomes the total to the all answer including multiple selections.
• The Chairmanship cannot be transferred until the result is closed.
